Credentials for LPN/Care Manager: LPN Licensed Social Worker Certified Case Manager (CCM) Job Experience Must have a minimum of 3 years of relevant care/case management experience Complex Care Management/Case Management/Management Care experience Community based or In-Home visit experience Experience working with Geriatric, chronically ill or functionally challenged populations You must have required documents below before applying. Thank you. * Copy of Resume * Passport size Pic for Care Manager ID badge * Emergency Contact Information * 2 Professional Reference * 2 Personal References * Driver's License * Social Security Card * LPN License and other qualifications * Recent Criminal Background Report * Current TB Test report * Drug Screening Report Job Description/Responsibilities: - Provide client service reports by documenting every patient visit using secure internal software - Provide care management services to clients and families - Assess client needs for psycho/social/functional status in the location. - Design and implement a care plan tailored to the needs of the elderly/disabled patient and their family. - Monitor services plan including compliance with the care plan and psycho/social/functional status of the customer and their family. - Facilitate placements into assisted living and/or long term care facilities, as needed. - Advocate on behalf of the Client with any service provider, caregiver and family members. - Assess home environment appropriateness and its impact on the plan of care. - Reduce isolation and stress of caregivers. - Provide quality care management and other services to Clients. - Provide the appropriate distribution of the local caseload based upon expertise, and time availability. - Provide availability of Care Manager Personnel to allow for coverage 24 hours per day, 7 days per week for crisis situations. - Maintain customer records including the appropriate invoicing for services rendered by the Care Manager, using formats provided by agency.
